Notice to Guest — Please Read Before Buying a Membership
- Do not sign this Contract before you read it or if it contains any blank spaces.
- You are entitled to an exact and completely filled-in copy of the contract you sign. Keep it to protect your legal rights.
- Under the law, you have the right to prepay your entire balance at any time with no pre-payment penalty.
- Under the law, you have the right up to a period of 1 year of your final payment (or such longer period as may be required by law), upon written request, to receive a written statement with (1) the amounts and dates of your payments, and (2) the total unpaid amount, if any.
- This Contract is covered by federal and state laws and you have the rights of a buyer under such laws.
- Member may terminate this EFT Agreement by providing written notice to Amelie Specialized Aesthetics at least 30 days in advance of the next scheduled payment date. In the event of termination, Amelie Specialized Aesthetics will not initiate further EFT transactions after the notice period.
- If there are insufficient funds in Member’s bank account to cover a scheduled EFT payment, Member may not be eligible for Member benefits until past due payments are made.
- Amelie Specialized Aesthetics reserves the right to make changes to the membership program, including membership fees and services, upon providing Member with 30 days notice.
- Amelie Specialized Aesthetics will take reasonable measures to ensure the security and confidentiality of Member’s bank account information.
- This Agreement constitutes the entire understanding between the parties and supersedes any prior or contemporaneous agreements or understandings, written or oral.
- All sales are final and non-refundable.
CAUTION — IT IS IMPORTANT THAT YOU THOROUGHLY READ THIS CONTRACT BEFORE YOU SIGN IT.
By signing, you confirm that: (i) you have read this Contract, (ii) you represent that the personal information you have provided in accordance with this Contract is accurate and authorize the use of such information for the purposes described in this Contract, and (iii) you have received a fully executed copy of this Contract.
